01Sensations during surgery

During the actual procedure, was there really no pain?

We gathered first-person accounts of sensations during surgery. While some reported feeling no pain, they noted that having the eye fixed and watching the laser felt terrifying. Another patient shared that the right eye did not hurt at all, but the left eye continuously stung. Even for the same individual, each eye could feel noticeably different. Rather than settling for a generic “it didn’t hurt,” it helps to see the exact moments and which eye each sensation involved.

02Tension and communication

If fear outweighed pain, what helped calm the nerves?

We looked into accounts where patients felt anxious aside from physical pain, but were reassured by ongoing explanations. One person who found the eye-docking step frightening said the calm, steady voice of the team made a significant difference. Another patient who had SMILE Pro combined with corneal cross-linking (CXL) also noted the reassuring verbal guidance throughout the procedure. Of course, feeling reassured does not guarantee complete absence of pain. This highlights the value of naming what you feel anxious about in advance and confirming how to signal discomfort mid-procedure.

03After the anesthesia wore off

Even if surgery felt fine, did things change back home?

We tracked separate accounts of stinging and burning after the procedure concluded. Some people had zero pain during surgery, but once the numbing drops wore off, their eyes felt raw, stinging, and burning. Another patient shared that watery eyes and sharp stinging began immediately after, settling down significantly only after about six hours. It is crucial not to assume how you feel on the operating table will carry over after returning home. Note that transportation methods or whether someone was accompanied were not consistently stated across reviews.

“it doesn't hurt during the surgery, but once the anesthesia wears off, your eyes will sting and feel irritated,”

04Smarting, tearing, and stinging

How long did the tearingand stinging actually last?

We compiled reviews detailing specific sensations such as stinging, tearing, and raw irritation. One patient who underwent SMILE Pro noted about three hours of tearing and sharp stinging, followed by dryness and glare around the one-week mark in that same account. Relief from stinging does not automatically signal that all post-op discomfort has ended. Because reviews omitting duration were also included in this cohort, the total count should not be read as the number of people who improved within a set number of hours.

“the pain, though my eyes teared up and stung for about 3 hours after the surgery, quickly subsided.”

05When pain eased up

When people say “there was no pain,”which timeframe do they mean?

We looked at claims of little to no pain alongside their specific timelines. One reviewer documented almost no pain by the second day. That timeframe is distinctly different from feeling fine during surgery but experiencing stinging once anesthesia fades. Reviews providing an overall pain score without a timeframe were also retained here. We avoided equating comfort on day two with an easy day one, nor can we predict identical progress down the line.

06Procedure duration vs. clinic stay

If the surgery takes 10 minutes, can you leave the clinic just as quickly?

We reviewed cases noting both brief surgery times and the full span spent on testing and waiting. One patient stated that evaluations took 30 to 40 minutes, while surgery itself was roughly 10 minutes. This represents a single person's visit log, not a guaranteed total clinic duration. Waiting intervals and post-arrival discomfort back home are separate matters altogether. A fast procedure alone cannot predict how the rest of your day or pain levels will unfold.

“The examination took 30-40 minutes, and the surgery seemed to take about 10 minutes.”

Why we chose these six points

SMILE and SMILE Pro terms have been retained as originally written. Cases combined with corneal cross-linking (CXL) or corneal reinforcement are explicitly noted and not interpreted as outcomes of laser refractive surgery alone. This section regroups relevant expressions extracted from the existing pool of 2,012 evidence-based reviews. Posts noting an absence of symptoms and those where timing is unclear are also included; the count in each section does not represent the number of individuals who experienced symptoms. Complex combination procedures and posts deferred from the initial screening were excluded from this reclassification. Modoodoc sources have been excluded from the analysis, quotations, and evidence database. All reviews assigned to each topic are linked without volume caps. Representative quotes subjected to source-text comparison are differentiated from automatically classified evidence; this distinction does not imply individual manual verification of the entire classification set. Consultations and pre-op examinations are designated as reference material. These figures do not indicate total patient volumes or clinical incidence rates.
